You can use the Acrobat Forms tools in Acrobat 8 Professional. Tools: Forms: Text Field tool.
If you want a Reader user to be able to add text and save the PDF, you will will need to "energize" the PDF using Advanced: Enable Usage Rights in Adobe Reader.
