If I use the Date Format Category on a pdf form text field how do I also display that date in uppercase? I have a date field that needs to be display in the format 'JANUARY 27'.

To use a different date format than what is included in the pulldown in Acrobat you will need to use JavaScript to format the date field. Have a look at this article on the JavaScript Corner here at AUC- it has an example for changing to Uppercase.
