Acrobat Form in LifeCycle Design "This operation not permitted" because of page numbers

I built a form in Acrobat 8 LiveCycle Designer from a converted (and combined of several) MS Word (2003) document. When I enable "Enable Usage Rights...", save and open in Acrobat Reader (8.0) I get "This operation not permitted". It appears it has to do with page numbers because there is a box with a question mark next to the page numbers. Is this a bug or something I can correct?

Hi jimmrz,

Can you confirm that this was your process for developing the form: import Word 2003 files into Designer, open PDF in Acrobat and Reader Enable, open PDF in Reader (which is where you get the error).
When you imported the Word file to Designer, did you get any errors in the log file? Did you get the question mark during PDF Preview in Designer?
What are the Document Properties associated with your PDF form (Ctrl+d)?
Also, have you tried using Save As in Acrobat before opeing the file in Reader?
