I saw numerous post about creating subforms or repeating a row if for example you were creating a purchase order. Well since I'm in the health profession I need to repeat an entire page. I'm wondering if the same tasks applies.
Here's my dilema. On the website below I posted a form that I created in Word 2007 and converted it to a PDF form. I'm using Acrobat 9 ES. I used the form wizzard in 9 to have it automatically detected all my form fields, which includes check boxes and tables. You will see that it did part of it but skipped a lot of it. That's another issue. Once I have all the fields I do the rest of the design in Livecycle ES 8.2.
If you look at my form after page 4 where you see where employees sign you will see from page 5 to 10 sections that are repeated. The reason for this is that a patient might have more than one problem so we have to use another sheet to indicate the patient's problem. BUT WHAT IF???? the patient only has 1 problem then I don't need to use pages 5 through 10. BUT WHAT IF???? the patient has 3 problems which means I need to use page 5,6,7.
Does subform scripting apply to my situation? So for example patient has 1 problem so the employee only needs to fill out pages 1-4 save it and then print out. If the patient has 3 problems can I put an input option button that asks a question, how many issues does patient have? If i enter 3, then the 3 extra pages appear.
I'm trying to reduce waist of paper that's printed. Even though I've showed employees that if they're only going to use pages 1-4 go into the printer properties and choose page 1-4 instead of the whole document.

I looked at your form. I attempted to load the .pdf into LCD ES. What you have created is a LiveCycle Designer version of a real PDF file.
My advice is to start over and design it all in LCD. This will allow you to get that repeating subform you desire.
