Beginner - Form questions?

I have a pdf form that I am trying to make corrections to. It is copying information you type into one cell into another cell.

I have never created a form pdf and am unsure exactly how to do this. I have tried to figure it out in order to figure out how to make this correction. I'm wondering, based on information I keep coming across, if I am in the wrong version of Acrobat. I am in 5.0. Do you need 8.0 to make a form pdf?

Acrobat 5 does not inclue LiveCycle Designer, but uses AcroForms. If you are copying fields and placing them as needed, you need to rename the copied field since any text field with the same name as another text field takes on the value and properties of the other field.
