Data disappears when pdf is opened.

I have a drop down field that selects an industry name. This also populates two other fields - permit number and street address. There are also two signature fields, one for author (does not lock fields), one for reviewer (locks fields). When the test document is signed by author, all appears well until the form is opened by the reviewer - street address has vanished, but permit number is still there and correct. The same problem arises when signed by the reviewer.
 
I would REALLY appreciate any help on this.

UPDATE: I don't think this has anything to do with the signature fields - just tried completing the form and saving it without signing, and I get the same result.

See if this setting that you make in LiveCycle Designer makes a difference: File > Properties > Defaults > Scripting > Preserve scripting changes to form when saved > Automatically

UPDATE: When I open the saved pdf, if I then re-select the industry name, the address re-appears. When I save it again, it does not keep it.
