I've had so many issues with this seemingly simple problem - so I'm hopeful to find the fix here!
What I am having difficulty with, is my global binding on the Master page. I have two fields set outside the content area, independently named. The binding for these two text fields is set to global. These two fields need to act like a header and repeat the user-filled-in information on ALL pages.
Despite my best efforts...when using the form with Reader or Pro, when typing in these two header fields, the information disappears and does NOT continue on subsequent pages. I've tried subforms, no subforms, making it a table, placing it in a new content area, etc. No luck. The only way the field works is when it's placed on the Design page (but I MUST have it display on the Master).

When I preview the form and type into the two text fields, their respective values are propagated on all pages, as expected.
My colleague Mark Leyden had a comment about the data format of your text fields which could be causing the problem. Please see [url=http://www.adobeforums.com/cgi-bin/webx?14@@.3bc4338b/8]his response to your post on the Designer Forum[/url] and let me know if that solves your problem.
Otherwise, what versions of Designer and Acrobat (or Reader) are you using?
