Distributing secure form

I would like to post an application form on the web, however it will container applicant health information. So I would like to have it be secure when it is e-mailed back to me, but would like anyone to be able to access it and complete the form. How do I do this.

As Thom notes in his post, you can send the form securely via email if you set security on the PDF first and then reader-enable the form in Acrobat Pro. If you want to return just the data via web submission you can use SSL.
