I would like to build a form with two fields.
Field 1 = ACCOUNT NUMBER
Field 2 = Patient Name

I would like to be able to enter in an account number and have my patient name populate from my MS SQL database.

You might like to take a look at Stefan Cameron's tutorial on the subject. This will give you the basis for loading in the fields from the SQL records.To pick a specific record (search), you simply need to check Stefan's selection explanation!Hope that helps!
