Grouped Table Header Fields

I'm new to Adobe Forms and can't figure out what is the best way to achieve grouped header. Currently I have 2 table headers one under another. First header shows groups and second one shows individual table columns.
The problem with this approach is that it seems not allowed to have "Include Header Row in Subsequent Pages" indicator set for both headers (the table is placed in a flowed subform).
What can I do about it?

You can group both header rows into a section, which then can be set to be included in subsequent pages.

Yes, I already tried it before, however, didn't mention it.
When I group the headers into a section I get an error while trying to preview or print the form in SAP, although in "Preview PDF" mode in Adobe CycleDesigner there are no errors.
The error that I get in SAP is: "ADS: com.adobe.ProcessingException: com.adobe.ads.exception.TransientException: A problem was encountered with the results: RenderResult array is null.(200.101)"
The trace file shows:
"Begin RenderAll CORBA call to XMLFormsModule elapsed time = 15 ms.
A problem was encountered with the results: RenderResult array is null.
A Render RETRY attempt should be made from calling system.
"

I'm not familiar with SAP version of Designer.
Another apporach it to merge all cells in the header into one cell, then change the field type from text into subform and adding all fields needed for the header into this subform.
By this mehthod, you don't need a second header row.

I believe, there is no difference between Designer version shipped with SAP and the standard one. And the issue that I have is not with Designer (I can preview my form successfully in it) but rather with ADS (Adobe Document Service), when it tries to generate a PDF-document.
