How do I resize Checkboxes?

I am trying to create a fillable copy of a medical form that we use for our screenings, and I am extremely perplexed about how to add a checkbox type of clickable object such as Question 1-15 of the the link (http://www.nhyoko.med.navy.mil/pdf/NAVMED01300-1.pdf). Does any one have any ideas on this?

Did you have a question about resizing checkboxes?

A check box is a standard type of form field in both Acrobat and LiveCycle Designer. Are you able to create other types of form fields?

That particular form allows you to simultaneously select Yes, No and N/A for each question, which does not make sense in most cases. There's an article here that you may find relevant: http://www.acrobatusers.com/tutorials/2007/js_radio_check_box/
