PDF Form Created in InDesign - Need to Merge with data for mailing

I have a form that was created in InDesign and then saved as a PDF. I am needing to mail this form out to about 1500 people and am looking for a way to do something similar to a word mail merge. I know acrobat is not designed for this purpose, but at the same time, I cannot recreate this form in word, as this form is the same form we have used for years and want the formatting to look the same.

Can anyone help me out? I am using Acrobat 8 Professional, CS2 for InDesign and am using Word 2007.

You can connect a LiveCycle form to a Database, so you can actually use this DB, and a little scripting, to create a variable data email. I covered the details of how to create this script in a session at the PDF Central Conference in September. I think they still have my slides posted, which includes some sample docs and code, www.pdfcentralconference.com. The session was "Practical DB connected forms".
