Show/Hide fields

I have a form with two checkboxes called US and CAN

What I am trying to do:

I have 7 fields on the form.
If the user select USA only 4 fields will show. Others are hidden
If the user select CAN only 3 fields will show Others are hidden

If unchecked fields will reappear.

How do I do this.?
I'm using FormDesigner 8.0
