submit button using the field name

Hi

I am totally new to this, creating my first form at the moment.

I have input field for user to enter NAME and EMAIL in the form.
I would like to create a submit which can do the following,

1) save the form as pdf (it will be great if it can be saved as 'NAME - the xxx form' by default)
2) send the form to a default email, and cc to the user (using the input EMAIL)
3) in the email, save the subject using user input NAME plus a default sentence

I have already spent a few hours and still have no clues, can someone please help?
