When I receive XML file for completed survey and import, I am missing information

Hi..I am so new to this! I created a form in Professional 9, Live Cycle Designer. It is a simple customer satisfaction survey for my department. There are questions on each side of the sheet where the customer can simply check a box to rate their satisfaction level.
 
I have a "Submit by Email" button, and the results are sent to me via email. I save the XML file in a folder, and then use it to import into the form.
 
When I view the form, only a few questions populate with the information. I have tried submitting dummy survey results, and all the information I am checking is just not showing up when I import.
 
Any thoughts on how I can get all my information to populate a form when I import? Any input would be greatly appreciated!!

I got it resolved...I looked at the file and saw that all the fields had the same name, so I created a unique name for each one, and now it works..
