Blank html upon export

I created a form in Lifecycle Designer, opened it in Acrobat 8 and then exported it as html so I could use it on a web site. But the html page is blank. I tried fooling around with the various settings on export, but nothing makes a difference. I have no idea what I am doing wrong. Do you? Thanks.

Have you tried saving the form in Designer as an Acrobat 6 Static PDF form? I don't think many of the form capabilities map over well to HTML but you may be able to pick up the formatting.
