Bookmark is added when I insert a file

I create PDF books from Word docs using automatic bookmark creation, then add a cover by inserting a one-page PDF at the beginning of my published book. In the past I'm I've done this without having any effects on my automatically generated bookmarks, but when I try this now, I'm getting a strange two-level bookmark added automatically to the end of my bookmark list. The bookmarks have the following format (where "my_cover.pdf" is the name of the inserted PDF file.)

[] my_cover.pdf
|_ [] Page-1

The cover pages that are producing this problem were generated by saving a Visio file as PDF. This is a new process for us. Is there something I can do to prevent these odd bookmarks? They are easily deleted, but I worry about missing one.

Check if the file you're inserting has a bookmark in it. If so, remove it and use that version of it.

This is known issue starting with version 6 of Acrobat. See [url=http://www.planetpdf.com/developer/article.asp?ContentID=RemovingfilenamebookmarkscreatedbyAcrobat&gid=6881]Removing filename bookmarks created by Acrobat[/url] by Sean Stewart for a JavaScirpt that can remove the added 'fieldname.PDF' book mark but retain all the bookmarks in both files. You will have to run this script through the JS Debugging console, but you could use this script as part of a new menu item or tool bar button.The script moves all the bookmarks below the filename.PDF and then removes the .PDF bookmark, so the bookmarks are retatained.
