When saving a document to a PDF file the image is not saved as 100% it is reduced in size. How do I make sure the document is full size.

What type of document are you starting with, and how are you doing the conversion? (in Acrobat or in the native application)

Generally it would be a word document being converted and I use the adobe pdf tool on the tool bar to convert.
