Is there a way I can create geotiffs from a geopdf so I can import the map into a project using a non-ESRI, non-terrago product? I don't mean just a print to tiff and register it myself, I mean make a tiff and a tfw file or a geotiff automatically.

In short - no. You cannot extract the georegistered map annotation from a PDF document.

So, no way to "Print to GeoTiff" or "Create GeoTiff"? Thats a bummer because I need some of the USGS quadrangles, and they only offer geopdf from their website. Where can I put in a feature request?

There's free geoTIFF data for most US states, but stashed on their own websites rather than USGS' central servers. Here are [url=http://gis.utah.gov/usgs-1-24k-topographic-maps-drgs/by-quad-name-usgs-scanned-topographic-maps-1-24000-geotiff-2]all the Utah quads[/url] for example. You just have to Google a bit. Some states are better than others at hiding it under a plague of sub-menus though!
As the USGS site sells their data as well as giving it away, it's their choice to make the free quads difficult to import into GIS software by delivering them as PDFs - the public are happy with something they can print out and stuff in a pocket, and professional users can buy the discs. Non-pro users also get something they can use in Reader 9 without having to install any GIS software, which was a complaint about the geoTIFF library - to 99% of people it was just a picture, and many couldn't even open a TIFF file.

You can get geoTIFF files for all states from one server. I think the files are on the USDA NRCS Geospatial Gateway and/or on the USGS TNM Viewer. However, if you already have the GeoPDFs, you should look into Global Mapper. You can download a free trial.
Global Mapper allows you to import GeoPDFs and most geospatial PDFs. Global Mapper also exports geospatial PDFs. The cool thing about Global Mapper is that it not only imports the USG's topo maps, it can also remove the white space or collar around the map. The program costs somewhere around 300 dollars.
