I want to put a document into a MS Word type state to modify and then save back to pdf - is this possible?

If you have Adobe Pro then you can publish back to WORD, make your changes and then convert back to PDF. However this only really works well with simple documents.

It can also work with more complex documents providing you can keep the document structure.
In other words, setup tables and bulleted lists, use reliable fonts and convert using the PDF Maker (from Word, IE) etc., etc.
By having structure inside the resultant PDF, exporting back to other formats proves more reliable. Acrobat 8 has much improved in this area.
