Hyperlink in PDF

How do I place a hyperlink to a URL so that the URL opens in a new browser window. I have tried everything (as per documentation and previous reply here to another similar query) but what I get is an object that attaches to the end of the PDF document but not in a new browser window. Also same goes to open a "mailto:" link. Thanks
