Inserting document issue

I'm having an issue with inserting documents-specifically, the problem is it will merge the writing on the documents together right over the top of each other. Help!

Acrobat Pro 9.0, Windows

Were these documents created in Acrobat or another PDF creation application? You can check this under the Document Properties (ctrl + d Windows). Do the documents contain form fields?

These documents were created using Acrobat version 9. They do contain fields that can be filled in either in Acrobat or Reader-is this what you are referring to?
