Menu bars, links and how to execute a menu item...

I am using Acrobat 7.0Pro to put the contents of a large book into CD format. I need to add a menu bar for users to turn the page forward, backwards, go to a contents page, and to exit the program. I have a few issues...

a) The pages of the book are provided to me as pdf files. The pages are top to bottom full of text, graphics, headers/footers, so to add a menu bar I need to add it outside of the existing pages or it will cover up some of the page content. If I create a menu bar image in photoshop/illustrator, how do I import it into acrobat and have it attach to the side of the page (imagine like a tab).

I don't have permission or access to change the original book.
Also,
b) I have played around with adding a button field to an existing graphic in my book, and when I go to the actions tab and select 'execute a menu item', I click add and get a message saying 'please select the menu item you want to execute from the menu bar' and an option to cancel. Should I be seeing a menu list, I don't understand what menu bar it refers to? I want to be able to exit the program, so confused about how to do this...

These seem such basic questions, but haven't had a need to use acrobat for this type of work before. Would be grateful for some help. Thanks.

Check Acrobat's JavaScript documentation, you will have to have each user add a file with your code to one of their Acrobat's/Readre's application JavaScripts folder since the add menu item method can only be run through the JavaScript Console/Debugger or during initialization.
