Try resetting your Navigation Panels from under the View >Navigation Panel > Reset Panels menu.

Do you see the Form icon in the top left corner when you're in Form Editing mode? If so, have you tried clicking on it?

Do you see the Navigation pane when you're not editing a form? Does it appear if you press F4?

Does this occur with all forms or just the one that crashed? If it is all your forms, you might try doing a Repair Acrobat Installation from under the Help menu.

As a follow up, this issue was related to a PDF that was damaged. Because the PDF form was damaged, the fields did not appear in the form navigation pane.
