Hi,
this behavior is very strange but reproducible an several Windows XP computers with Acrobat 9 (Pro or Extended, even in 9.4.6)in our company.
 
The thing is, that the pagenumbers are incremented after the optimizing process.
This occurs at every file on. It is not the problem of a special file!
The improvements are also settings regardless.

Is it a bug? Do we have problems on our machines?
