Many times I have electronic patient records that need to be signed whenever I receive a release request. Is there a way to create an automatic function within acrobat x standard to sign every time the field below is listed on the document?

Is the field name the same on all forms?

Yes, it is. When I pdf the chart, the weekly sessions will have the same fields for signing.
