Acrobat Stamp Dynamic Text

I process a lot of invoices for various people and need to put a stamp on each invoice (the easy part), but, this stamp needs to have various items on it (the hard part). I need an "approved by" section with a checkmark box to the left of the "approved by" text. To the right of this text, after the check box is checked, I need a text box where a cost code can be manually entered so that I now what job to charge. I also need the check box to activate another text box below the "approved by" text where a dollar amount can be entered (we don't always pay the full invoice amount due to money retention issues).

I've been reading quite a bit on how to create these "dynamic" stamps, but nothing I have read covers anything quite like this. Is what I'm trying to do even possible? If it is, has anyone found any information on how to do it. I realize that it might be possible to do with some intense javascripting, but I'm not a programmer and have no experience with this kind of scripting.
