add-in option in Tools pane

Helle everyone,
 
i recently copied some JS from AUC to add some custom menu items and it was working fine. i added a couple of items to the end of the File menu. today they are not showing up. i also noticed a custome TLB i created is gone. the JS file is still intack and in place. i did notice that the add-in menu item, under the Tools pane, is missing. i remember having to select or include the add-in somehow, but i cant remember how or where the add-in option is located.
