How would I create a series of independent PDFs that an individual can fill in a quantity, and have the selections from each form collected into a list single pdf list that can be reviewed and then submitted?

You can use the "Distribute Form" feature in Acrobat Pro. 8 to send the forms out via email. The data will be returned to a location that you specify. You can then compile the form data or merge the data into a spreadsheet. You can also use the Form Tracker to determine the status of each form that you distribute.
