You need to create a data connection and it's something that we can't easily explain here in this forum.
Your starting point would be to use Adobe LiveCycle Designer (Windows only) to create your form and create the data connection. Look to the help and sample files installed with Designewr, on Adobe's Web Site in the LiveCycle forum, and also the online support you can access from the Designer Help menu.
