A Job for a Java Script Expert

I've created a form in Acrobat... the problem is there is a message that pops up with every field saying "Warning: JavaScript Window: The value entered does not match the format of the field [ member_retention ]". The calculation (just one!) works, but the form doesn't.

I need to get this completed and out for my job. I need someone who can trouble shoot this for me, and get the form to work. And we're on a tight schedule - it needs to be completed by the end of the week.

What I'd like to do is have you let me know you are interested; then I'd email you the form, and you can take a look and then we could talk about what would be involved and set a plan. We can also discuss compensation for your work.

The form was created in Acrobat 9, on a Mac...
