Need help with Javascript and dynamic stamps

I'll start off by saying that I know absolutely zero about Javascript, and very little about dynamic stamps. I'm trying to create a dynamic stamp that when the user selects it a popup box will appear and they will have to check off one of 4 radio boxes, and the current date is inserted into the stamp. I found a stamp on pdfscripting.com (I probably shouldn't post the script as I had to pay for membership to the site) it does exactly what I want it to do, but I need that functionality on a specific stamp, not the generic one from the website. I've tried copying all the fields of the stamp and the javascript over to my custom stamp, but nothing works, I don't get any popups or the date isn't inserted. What am I doing wrong? All I want is to copy over the example that I found and modify it a bit to work with the stamp. Any ideas? This has been driving me crazy for three days now. Thanks.

Instead of copying things from the stamp file that's working to one that isn't, try replacing the page in the working file with the one you want to use (Tools -- Pages -- Replace). You may then have to rearrange any fields so they're in the right locations, but it should work.
