Popup A to Z list which links to bookmarks

Hi there, I am a novice user trying to get started with active pdfs.

I have a client that would like to have a popup A to Z list on the contents page of a document that would link to bookmarked pages within the same pdf. Any help or pointers would be most appreciated.
