My CPA firm wants to repalce it's paper files with PDF's I am looking for the best way to make file folder templates that make it easy to scan the paper into bookmarked and indexed sections that are setup like are paper files I have searched and not been able to find much help

And you won't. Both bookmarks and indexes are the kinds of things that you add after scan and OCR. After all it's a little difficult to create an index until you actually have the text to be indexed. However, this is something you can do. First you have to create a PDF with the bookmark and index page that you want. Then when you scan, specify that the scanned pages are appended to the current document.
