Job Offered: Develop a fillable form with some uncommon functions

Small company requires the development of a fillable form for use on Tablet PCs running Windows XP and Acrobat READER v9. The form is one-page and is not very complex, but requires the following:

1) The form must be able to capture and save signatures (the traditional kind) that are provided via a stylus. The date and time that the signature is captured as a non-editable data field in the form.

2) The form must have a submit function which creates an e-mail with the saved .pdf form (with its data inputs and embedded signature image) as an attachment. The subject and message body of the e-mail are generated based on user inputs in the form (unformatted text only). The only e-mail client on our machines is Lotus Notes v8.5.

3) The form and its data inputs must be save-able by the user at any point before submission (so that user can partially complete form and return later to complete it).

I can send full specifications if you are interested in this project. Please only express interest in this project if you are confident that you can build the aforementioned features into a form without difficulty.

If possible, we are looking for a quick turn-around time on this project.

It should not matter, but, for what it is worth, I am based in Toronto, Canada. However, my company is international including an office in the U.S.. Payment for services can be in U.S. or Canadian funds.
