Job Offered: Scripted PDF form for funeral home group in Massachusetts

Many years ago I developed a couple of semi-automated Word templates using VBA. These need to be updated and in the process I want to make them platform independent via Acrobat. Unfortunately I don't have time to get back into development even though I enjoy it very much.
 
In it's simplest form it could be a multi page document which collects user inputs (combination of drop-downs, dates and text) on page one and then reconfigures the information in several different ways for other users who can print out the page or pages they need.
 
If possible, I would also want this to be compatible with PDF Expert on the iPad. I know that App supports form fields, but I'm not sure how extensively they support scripting.
 
Right now this information is handwritten, faxed and then transferred to the forms in our office. I want to put a tool in the funeral directors' hands which they can fill and distribute electronically without requiring a database backend.
 
From my limited research it seems a document like this is plausible and I'm interested in finding a developer who can flesh it out fairly quickly.
