Hi,
We've got a few page Word document that we want to convert to Acrobat. It has a bunch of fields, checkboxes, etc. and we want to make it pdf-fillable. Might even like to explore having some sections automatically fill into other sections (is that done with java?)

Please respond if you can help. For those I get in touch with, I will ask to see examples of your work as well.

Hi Steve. We offer a service to create Microsoft Word Forms, Adobe PDF Forms (Acrobat and LiveCycle) and Web Forms (HTML) that are integrated with database (Microsoft Access).

The forms can be translated in different languages and used for any data collection, including long questionnaire surveys. The data in forms can be automatically imported into the database with software provided. The data can also be exported in standard Triple-S format for use with statistical software such as SAS and SPSS.
