Acrobat 8 and Word Heading 2 problems

I am using Acrobat 8 and word 2003. Our template uses style refs that point to a Heading 2 so that the text of th heading 2 appears in the page header When I create a PDF using PDFMaker the last heading 2 in my document appears on all my odd page headers.When I create the PDF using File > Print and select the Adobe PDF print driver the headings work correctly but I do not get any bookmarks. What can I do to fix the problem.
