Can't get bookmarks pane to appear in PDF, and extra bookmarks are being added.

I create a Word file from RoboHelp and then I use the Acrobat plug-in to generate a PDF.
 
Up till now I had been having no problem generating bookmarks in the PDF. The bookmarks pane appeared in the output, with Heading 1 and 2 entries.
 
All of a sudden today the bookmarks pane is not appearing in the PDF. If I go to View > Navigation Panels > Bookmarks the pane will appear, although it is floating and I don't see how I can anchor it. Even if I go to Properties > Initial View > Bookmarks and Page, it will not show up.
 
Additionally, I have "Convert Word Headings to Bookmarks" selected and Heading 1 and Heading 2 checked off, but Heading 3 is also being included in the bookmarks.
 
Any help would be appreciated.

To get the floating bookmarks window to be docked again, click and drag on the Bookmarks tab, as opposed to the top bar of the floating window, and drag it back to the navigation pane area.
