HTML to PDF conversion

I need to covert a series of HTML files to a single PDF.
 
I've tried 2 approaches:
 
1. Open the index.php file in Acrobat. When I do this, all the links in the TOC frame under a book (series of links) are expanded. So, instead of index.php being one page, it's 14...not good. However, the hyperlinks to other pages are retained...essential.
 
2. Open index.php in a browser window (IE) and print to PDF. The index.php file (TOC frame) is one page...good but all the hyperlinks are lost...NOT ACCEPTABLE.
 
Am I missing something here? Is there a way that I can open index.php, get just the TOC on one page and keep all the hyerlinks?
 
Last, I don't want to click each of the links in the TOC one at a time, create its own PDF, and links them in Acrobat. The current PDF file is >700 pages and this would be crazy.

If you have your HTML files either on your hard drive or on a Web server, use he Create PDF From Web page command and set the number of levels in the Get only checkbox to a higher number than 1. Of course the more levels you identify the more pages will be converted to PDF and can take a considerable time. You can also edit settings by clicking the Settings button in the Create PDF From Web Page dialog box.
