Is there information (knowledge base? tutorial?) somewhere on this site regarding creating an eBook from a MS Word document?

I can combine three pdfs (front and back covers and main document). I am looking for guidance on creating a pdf specifically for an eBook reader. I have seen several references from users who are unable to see Word's headers (which contain page numbering), but none of these questions seem to be answered.

Are guidelines for formats for the different eBook readers available?
