Scanned Form - Printing Fillables Only

I have just scanned a particular DMV form. I want to create a fillable fields on the DMV form. I would like to print the fillable fields only. I am not interested in printing the scanned form.
 
I want to print fillable field on original authentic DMV form.
 
DMV only accepts original Forms. DMV does not care if the written parts is hand writing or printed.

Hi AFHCU,

In the Print dialog ( File ->Print) there is a pulldown titled Comments and Forms. In that pulldown choose the option "Form Fields Only."Hope this helps,

Yes, How do you permanently select Form Fields Only? So No one can modifie it.

That's a tricky one. I think you could add a script on a button that would bring up the Print dialog with that option (Form Fields only) as the default.
