Linking pages within pdf portfolio

I am currently creating a pdf portfolio and I was wondering if there was a way to create a link on a Table of Contents that when clicking on that link would take you to that particular document in the Portfolio. For example if I were to make a book where each chapter of the book was a separate document in the portfolio would there be a way to link my table of contents to take me straight to a chapter instead of having to click across the bottom until I arrived at Ch 15.
