PDF form -creating a submit button to email the completed form

I created a form using Quark Xpress on a Mac. Turned it into an interactive form (live fields). I'd like to have a submit button so a person could download the PDF, open it, fill out the form, click the submit button and have it automatically attached to the email and send it back to me. So I need the button to pull up the mail form with my email address and the completed form needs to be attached.

I'm on a Mac with Acrobat Pro 9. I would love some step by step instructions or a link that will explain the teps?

My Product Information:
Acrobat Pro 9.0, Macintosh

I created a submit button on a form and it works fine but when I have a coworker click it an error message comes up saying "operation failed". What would cause this?

ssworthi,

If you've set up the form to "submit" the PDF via email, this will happen if the Reader is used and the form has not been Reader-enabled.

I'm not sure how to do that; could you help me with that? Is it in form properties? Thanks so much!

Acrobat 9: Advanced > Extended Features in Adobe Reader
Acrobat 10: File > Save As > Reader Extended PDF > Enable Additional Features
