You can find this option under File / Print / Advanced / Mark and Bleeds / Page Information. This will take the file name and automatically place on the output.
If you want more control then use the Document / Add Header / Footer command (or indeed any of the ones in the top category) ..

I've been using Acrobat since v1.0 and still get amazed by its power. An Acrobat ACE since 1999
I have tried this but...'File / Print / Advanced / Mark and Bleeds / Page Information'. Is greyed out, not allowing me to make any changes. Any ideas why?
I've also tried adding 'headers' with the file name but it appears over the image area, I need it to appear at the top, outside the image area.
Its driving me nuts, any help would be greatly appreciated!!
Ahhh, I have made it work by 'printing' to another pdf. Not sure why our printers are not compatible, I have got my IT team checking the print privalages.
Only if the PDF has a form field and necessary JavaScript added to the PDF prior to distribution. Reader does not have pre-press processing, can not add watermarks, can not add form fields, nor add annotaitons.
