Can't embed Flash into a PDF doc

I'm using Acrobat Professional 6.0

ERROR MESSAGE: Cannot create the movie annotation because the movie could not be loaded. This can occur if the file cannot be found or the URL is unreachable (make sure proxy server settings are correct).

I have reviewed other POSTS with the same problem on Acrobat 7.0 -- put don't see any answer... I have the same issue on v 6.0:

I have confirmed that the file and path is correct (on my harddisk)

Settings:

Checked: Acrobat 6 Compatible Media
Using the Movie Tool, Acrobat 6 Compatible Media
Browsed to .swf file
Content Type: 'application/x-shockwave-flash'
Checked 'Snap to content proportions'
Checked 'Embed content in document'
Poster Settings: Retrieve poster from movie

Progress Bar: Initializing QuickTime

Then get the error message above!
I checked preferences in Acrobat: Preferred Media Player --
Macromedia Flash Player
I'm using QuickTime Pro version 7.1.3
File Types 'Default' (includes video)

ANY ONE HAVE IDEAS HOW CAN I GET THIS WORKING?

Double-click the Flash marquee on your document page to open the dialog box, and check the settings for the annotations.

I have a slightly different problem, possibly since applying patches 8.1.1/2
and installing Flex Builder 3(? it's been awhile since embedding Flash)

If I use the Movie tool, SWFs are not enabled. If I open an older PDF with SWFs already embedded and try to add a rendition by showing all files and selecting a new SWF, I get:

"The type of file you have selected is not supported. Please select another file."

And in fact, if I click on the SWFs they won't activate - Acro3D wants to download a plug-in, which I usually don't allow since I like to vet my plug-ins before installing.

There still seems to be a flash plugin in my ... plug_ins/Multimedia/MPP folder, and the other types of annotations still seem to embed.

Also, assuming Adobe hasn't intentionally cancelled Flash support in Acrobat, supposedly there's some way to get the Flash to start up without the annoying warning message - is this a matter of signing the SWF, or something else?
