My first time attempting this. How do I enforce view restrictions? Is it only possible with a DRM and dedicated server?

It is by far the best approach, especially if you need to be compliant with standards like HIPAA. Adobe's offering is LiveCycle Rights Management ES2, but you might want to look into LockLizard and FileOpen as well.
