We created a form in Adobe Acrobat 9 last year and distributed it to 17 schools in our area. They completed the form and returned it. We now want to update that form with some changes and redistribute the form with the information they provided last year already filled in. For example, we don't want them to have to fill in fields again where the information has not changed since last year (such as their school name, address, phone and a variety of other information.) We also don't want to have to make changes to all 17 forms (one for each school) in order to change the form slightly (adding or deleting a few fields). Perhaps there is some way to change the original form once and then be able to pull in each school's data and then distribute it to them? Do you understand what we are trying to do? Is there a way to do this -- or do you have another suggestion about how we might accomplish our goal to make it easier for the school (and us) by not having to retype information? Thanks for your help!!!
You need to create a unique group field name for each individual like:
Child1.Gender
Child2.Gender
Child3.Gender
And with in each group have 2 radio buttons one with and export value of 'm' and the other 'f'.

What happens if i have multiple gender fields? I want to make a form for a family with a lots of fields. Lets say a family has 3 children. I made 3 radio button fields:
Child1 - Gender: * Male * Female
Child2 - Gender: * Male * Female
Child3 - Gender: * Male * Female
These fields have the same name as in the tutorial: Gender
At the first child I can select the gender, but when I change the gender at of second child the first one disappears.
I tried to find something on designfindr.com. I searched for acrobat forms.http://designfindr.com/term/acrobat+forms
What can i do?
