What is the best way to extend the comment properties

Hi

I am part of a small taskforce who is trying to explore if the comments stored in the PDF documents can be stored in different meta data fields instead of a common text field. For example, while reviewing a document, reviewers might come across errors, which should be classified by a severity field along with the description of the error and may be a suggested resolution in another field. The inbuilt comment field in acrobat only provides a single text field to enter all this information.

I would like to find out if the comment properties can be extended using JavaScript or any other method such that the error or fault information can be stored in different fields.

If it is not possible to extend the comment properties window, what would be the best way to implement something like this.
