Allow users to attach file to PDF doc

I have a PDF file that I have been using to collect information from students. I created in Arobat Pro 9 and I have instructed my students to download Reader 9 to complete the form.

I now want to add functionality for students to "click on a box" and attach their photo to the form they have completed.

I have not been able to find a way to do this .... suggestions?

If it's just a photo then you can use the Image Field in LiveCycle Designer to create a button for importing an image into a field.
